Her lutz should have gotten a full -4 GOE reduction. e caps the GOE at +2 starting point. -2 to -4 is the allowable range of reduction for an e call. This is realty but uberring is not about reality.I think her growth spurts were the problem more then her being a headcase. I will always trust Isabeau more then Amber.

One of the requirements to get above a +3 to begin with is that the jump has to have a good take off and landing. Doing the wrong take off is not only not fulfilling that, but it should dock a criteria right there as it is.
Wrong edge 'e' is -2 to -4. You can't give a triple flip, intended to be a triple Lutz any kind of credibility as an actual jump. Should be -3 to -4 after all is done because there is zero attempt at trying a Lutz.
And no, it never changes from competition to competition. She does the same thing time and time again. To solve the problem, it should just be called a 0. If someone does a toe, Sal, or loop for the third time, they get a 0, lose the element block, and *. With Kaori's three flips, we give leeway because she 'meant' to do a Lutz... meant being very sarcastic here.
